Job Description
RiseWell Community Services is seeking a full-time Case Manager to join our Residential Department. This is a hybrid position, and in the role, the Case Manager will be reporting directly to the program supervisor. Job Function Provide case management services, housing supports, and coordination of services to people with psychiatric disabilities living in scattered apartment settings.
Qualifications High School Diploma and at least 1 year of experience in mental health. BA in Human Services and 1 year of experience preferred. Clean, valid NY State Driver’s License. Fingerprinting, criminal record check, and approval from the NYS Office of Mental Health.
